and where is all the leisure time i was supposed to look forward to? the reality is that because technology enables us to do things more quickly, we simply do more. we pack more work into our lives instead of allowing rest and relaxation to fill the extra time. life has become more complicated it looks, and we experience more, not less, stress. and stress, we are so often told, is a major contributor to illness: some sources claiming that up to 70% of all illness is in some way related to stress! (although, in reality, meaningful statistics are still sketchy on this subject there is clear evidence that work-related stress is on the increase.) in enjoying technology-led pastimes – computer games, social media, online chat – we move our bodies much less than a few decades ago. we are increasingly sedentary. and that, we are told, is a major contributor factor to the rapid rising incidence of overweight and obesity, which is reportedly reaching “epidemic” proportions and increasingly linked with serious illnesses such as diabetes, heart disease, cancer. it is not just information technology that has advanced at a massive pace in the past 50 or so years either. food technology has revolutionized the way we eat. my formative years coincide with some of the earliest “convenience foods”: remember the dehydrated vesta curry? boil-in-the-bag fish, doused in ready-made sauces? angel delight and other powdered desserts in foil sachets? take a walk through any modern supermarket today and you'll see how much shelf space is taken up with manufactured / processed foods, products containing artificial ingredients, chemically modified foods – reduced fat, de-alcoholized, hydrogenated. now, it emerges, processed foods are the least healthy options: in 2009 which magazine demonized breakfast cereals, suggesting that they actually belonged in the biscuit or confectionary aisles due to their disproportionally high sugar content; in 2012 harvard university branded manufactured low-fat foods dangerous, calling for an end to “the low-fat myth”; in 2013 processed meats are now finding notoriety, recently reported as a major cause of serious illnesses. then there are advances in medical technology. certainly i have witnessed modern miracles! dr. christian barnard pioneering the first heart transplant in 1967; the birth of louise brown, the first “test tube” baby in 1978. too many brilliant surgical advances to mention! but not all medical advances have been beneficial. we have also seen the rise of pharmaceuticals to prominence in modern healthcare. so many “wonder” drugs have let us down: think thalidomide; hrt, initially hailed as a lifelong solution in women's health, but intrinsically linked with breast cancer just a decade or so later; aspirin, a seemingly benign painkiller now known to have serious gastric consequences. this is to name just a few examples, yet the lesson has not been learned. my heart sinks every time the media hails a new “miracle cure” or “wonder drug”: we continue to believe the hype without real evidence. no doubt in a decade or so we'll find out exactly why everyone over 50 should not be taking statins! it is my hope that in the second half of my life i will witness the resurgence of the natural over technology: because we now have the opportunity for retrospect! that said, i do not want to deny all the great accomplishments of the past few decades – and there have been many. but i do want us to learn from our technological mistakes – and there have been many. the greatest mistake we have made, and continue to make, is to render obsoleste our past achievements. a fine example of this is the case of hippocrates, who famously said “let food be thy medicine”. doctors in many countries no longer swear the hippocratic oath: they have rendered such wisdom obsolete! ironically “retro” is trending right now! small groups of people are honoring past achievements as a result of hindsight. vinyl records, over cds or mp3s, enjoy a cult following. 60s fashions, 70s fashions, 80s fashions have all enjoyed their respect come-backs. if our society can honor the arts of the recent past, what about the sciences? do we have the courage to admit that natural bowel cleansing has the edge over chemical laxatives, now known to cause irritation, inflammation and to damage long-term bowel health? do we have the courage to promote “natural” therapies such as acupuncture over remedies such as nsaids, now linked with long term, even irreversible, gastro-intestinal damage? do we have the courage to correct our dietary habits to halt the burgeoning obesity problem or the diabetes epidemic or to prevent future health crises? the retro medicine mission aims to do just that: to honor traditional and natural therapies and all forms of medicine that hindsight shows do work, and to challenge the un-evidenced, profit-driven aspects of modern medicine that dominates our current healthcare landscape. someone once said “hindsight is 20/20 vision”. let's have the courage to trust what we can already see perfectly! { comments are closed } time for the “retro medicine” revolution by admin on june 12, 2018 ok so this is a term that i've made up! you will not have heard of it before, because this is the first time it's ever been mentioned. so i know that even though you are a smart, intelligent, thinking man or woman it's ok for me to teach you what i mean by “retro medicine”. for 13 years i've practiced what most people might say is “traditional” medicine, having initially qualified as a traditional chinese acupuncturist. that's really an umbrella term for several modalities of treatment – needling (of course), acupressure massage, cupping, moxibustion, traditional chinese diet advice, lifestyle advice, relaxation techniques, exercise and body movement techniques. over the years i've added to my training and qualifications to include colon hydrotherapy and nutritional therapy in my therapeutic portfolio, or tool bag. these both have ancient traditional roots too, and to appreciate their provenance think hippocates as the “father of modern medicine” who is quoted as famously saying “let food be thy medicine”; and think the dead sea scrolls and the egyptian hieroglyphics that depict and include instructions for inner cleansing of the bowel. recently i've been questioning whether or not what i do is truly “traditional”. sure my therapies have traditional roots, but i certainly do not practice any of them exactly as in ancient times. for example, long-necked gourds have been replaced by modern plastic disposable tubing and specula for colonics; i use modern turned stainless steel acupuncture needles; and my nutritional therapy practice, especially, is influenced hugely by modern clinical research. the recognition that what i do is essentially very modern, involving up to date practices but without losing their ancient essences and maintaining their traditional benefits, has led to my current reflections. reflections that make me ask what is it that i really do? what is the thread that connects the various parts of my therapeutic practice? simply, what is my “thing”? i realize that i do not want to turn back the hands of time. i do not want to go back to ancient ways, and i do not encourage others to do so either. but also, i can not in good conscience agree with much of our modern western medical practices. i'm not an enemy of modern medicine. there have been some amazing medical advances that deserve and have my respect. but … and there's always a “but” … there is so much about modern medicine, particularly the use of pharmaceuticals, that is largely damaging. i challenge you to name a drug that does not come with a host of side effects – some mild, some terribly severe, with long lasting disparities far worse than the condition they aim to treat. am i being over dramatic here? i do not think so. there are so many examples littering the brief history of pharmaceutical intervention it's shocking. aspirin, once considered to be a benign, even safe, pain killer is now known to cause serious gastro-intestinal problems; hormone replacement therapy (hrt) originally considered to be the wonder drug that women could take forever was discredited just a decade later when it's role in the development of breast and other hormonal cancers become apparent; antibiotics that have been prescribed with impunity over a few decades are now known to injure the immune system and are tolerated so well by the pathogens that they aim to kill that we are faced with a modern epidemic of deadly antibiotic resistant strains of bacteria. what's more, pharmacies have no role in disease prevention – whatever their profit-hungry parent companies would suggest or have you believe! so here's my “thing” – i want the best of both worlds – the ancient and modern, the traditional and contemporary – for myself and my patients and for you too! in taking a retrospective look at what traditional and modern medicines and medicinal approaches have done for us we can cherry-pick what helps and what hormones. “retrospective”? take a look in the dictionary and you'll find it means something akin to looking back, contemplating what's past. and “retro”? more about style – specifically of past times. and that's it! my “thing” can be captured in two words – “retro medicine”. “retro medicine” is my creation of a portfolio of clinical approaches for health and wellbeing resulting from understanding what works, what does not, what in retrospect has been proven to help, to be effective, to be truly beneficial. retro medicine has no place for bias towards traditional or modern techniques. it has no place for prejudice against traditional or modern techniques. rather, it will appeal to open-mindedness, critical thinking and reflection – the qualities associated with a genuinely intelligent mind. retro medicine is the pragmatic fusion of traditional and modern techniques that stand up to scrutiny and are shown clinically to benefit your health, promote well being, and help you to get the very best from your life. i'm here to start the retro medicine revolution! { comments are closed } some super simple tips on lowering cortisol levels by admin on june 12, 2018 some super simple tips on lowering cortisol. ok, so after my last article i've had lots of responses from people asking for some points on how to actually lower their cortisol levels. previously we looked at signs that you could have high cortisol levels. before i give you a few tips on how to reduce it, here's a quick reminder – cortisol is released in response to stresses in the body, its main roles are to suppress the immune system and raise blood sugar levels in preparation for fight or flight. .. so bearing these things in mind here they are: reduce caffeine – caffeine increases the secretion of cortisol, that's why it keeps us going. ' on top of this, experiments have shown that the higher your body fat percentage the greater level of cortisol is released – its a viscous cycle. get some more sleep – sleep is key to recovery, while you sleep your body is healing and detoxifying, importantly it is ridding your body of cortisol. if you're sleeping less than eight hours a night then you are already starting your day off with a dose of cortisol in your system. so get your head on your pillow before 10.30pm. understanding your stress – finding an equal balance is important, stressed mind + stressed body = raised cortisol. if your mind is stressing about something then the last thing you want to do is add to that by following an intensive training program. likewise, if you've just come from the gym the last thing you want to be doing is engaging your mind in a brain boggling sudoku. meditate – make a conscious effort to set a small portion of your day as for relaxation meditation or yoga. laugh – laugh regularly and laugh hard. laughing reduces cortisol as well as epinephrine (adrenaline). it also welcomes the release of endorphins – which gives you that feeling of a natural high. hang out with some funny friends or family, watch a comedy, essentially just have fun. take magnesium supplements – magnesium is used by the body to detoxify cortisol. the majority of us are majorly deficient in magnesium, it is no coincidence then because we stay highly stressed out. remembering that magnesium helps you into a relaxed state its best to be taken around bed time. go on a holiday – anywhere that helps you to relax, it does not have to be to the bahama's for two weeks, maybe a weekend away somewhere quiet where you can recharge your batteries. the point here is to disconnect from your working life and familiarizations that raise your cortisol levels. friends and family – spend time with friends / family, people who you are close to, avoid conversations about work and keep everything as light-hearted as possible. change your training routine – if you are training 4 times a week or more for extended periods of time, your body is likely to be in a state of fight / flight. try lowering who are interested in becoming home health aides should be at least 18 and have clean backgrounds (backgrounds checks are common). depending on your state, formal training is not always required. (physician’s office) by admin on june 12, 2018 over the last 7 years, we have provided consulting services to more than 100 different medical device startups. some of which have planned to obtain the ce mark for their product relatively quickly and prepared to launch it in germany. the problem was getting those german sickness funds (payers) to pay for it – and fast. in one of our previous articles, we discussed a possible reimbursements shortcut for medical devices that are being used in the hospital inpatient setting. we present in this article a reliably new option of securing interim reimbursements for devices that are also being used outside the hospital – in the physician's office setting. 1. the problem according to german law, innovative procedures and devices, utilized in the physician's office setting, are not reimbursed, unless they have been officially approved following a positive reimbursement determination by the federal joint committee (g-ba). unfortunately, an innovative procedure or device will not be issued a positive reimbursement determination from the g-ba unless it is in wide use in germany, has been thoroughly tested and comes with rigorous data demonstrating its value. obviously, prior to obtaining reimbursements it would be very difficult to obtain this criterion, and therefore innovative procedures and devices may never obtain reimbursements, which make a cycle unable to fulfill. another catch-22 … 2. the solution according to the recently enacted law for the restructuring of the statutory health insurance ( gkv-versorgungsstrukturgesetz ), instead of denying reimbursements from procedures or devices that do not yet fulfill the required threshold, the g-ba can now decide to provide funding for a ' controlled study ', during which time the above threshold may be achieved. funding for such a 'controlled study' may be granted to new medical diagnostic and treatment methods which present a potential for providing a better diagnostic and treatment method, compared with the current alternatives. the proposed process is currently under review by the german ministry of health and the effective date for the program will only be finalized once it is published in the federal gazette . 3. the process a. application a manufacturer of a medical device, utilized for the provision of a new medical diagnostic or treatment procedure, may apply for a 'controlled study' from the g-ba. the application includes the following 6 sections: section i – administrative information: name and address of applicant and contact person. section ii – summary: description of the suggested new medical diagnostic or treatment method, relevant indication, target population, expected diffusion in the german market and expected benefits. section iii – information on medical product: product description and technical instructions, comparable products, regulatory status and available clinical data. section iv – potential: the application should demonstrate by “meaningful documents” such as published articles of randomized controlled studies, the potential of the new medical diagnostic or treatment method as a treatment alternative. section v – key elements of proposed study: this optional section describes the type of the suggested study, relevant population, required sample size, appropriate comparative intervention, endpoints, duration and estimated costs. section vi – sponsorship and completeness of application the application itself is free of charge, however, in this section the applicant also signs a letter of intent, indicating its willingness to assert its fair share of the costs, such as the overhead costs associated with the 'controlled study', while the sickness funds pay for the provision of the medical procedure. the process also allows the applicable to submit the g-ba with a written request for a fee-based, non-binding consultation to provide guidance on the preparation of an appropriate application. the guidance should be provided within 8 weeks from submission and costs between € 500 and € 10,000, depending on its complexity. the information provided during the consultation is confidential. b. initial assessment within 3 months of submitting an application, the g-ba has to decide whether 'controlled study' would be appropriate and notify the applicant accordingly. an approval as this stage would typically require sufficient evidence, which demonstrate how the new medical diagnostic or treatment procedure presents a potential to replace currently available alternative methods which may be more expensive, more invasive, result in more side effects or feature any other disadvantages. in addition, it should be clear to the g-ba that the proposed 'controlled study' could generate the missing evidence in order to integrate the new medical diagnostic or treatment procedure into the healthcare system. c. study plan for the scientific monitoring and evaluation of the 'controlled study', the g-ba may commission an independent academic institute. the institute will be responsible for designing the 'controlled study', which may also include required quality criteria. after the estimated overhead costs of running the 'controlled study' have been assessed, the applicant will need to submit a declaration of financial support and assume the obligation to conclude a funding agreement between the applicant, the g-ba and the involved scientific institute. this agreement, signed prior to the initiation of the study, will describe who pays for what, how much and when. special discounts, up to 70% of overall costs, may be granted to small and medium size businesses (categorized according to annual turnover and number of employees) and also for new medical diagnostic or treatment procedures aimed at diagnosing or treating rare diseases. d. re-assessment following a successful 'controlled study', the g-ba may decide to provide reimbursements for the new medical diagnostic or treatment method. the average duration of the administrative process is estimated at 29 months (excluding the time period of the 'controlled study'). this may appear a long time, however it allows the company to validate its technology and provide the evidence to prove it using external resources. the generated evidence may also be used when approaching other markets, outside of germany. 4. conclusion this recent change in the g-ba's ability to perform more studies, instead of rejecting an application for reimbursements, especially in the physician's office setting, is good news for the industry. by providing a solution to the notorious 'chicken and egg' situation that so many companies have been struggling with,
